Provides primary assistance and basic needs assistance such as food, clothing, bedding and basic household items or small appliances. May provide diapers, baby clothing and baby furniture such as cribs or changing tables when donated items are available. In some situations financial assistance is given for rent/mortgage, utility disconnects, and prescription drugs. Makes bathing facility available to clients if needed.HOW SERVICE IS PROVIDED: On-siteWHO QUALIFIES: Client can only receive clothing, utility, rent and prescription assistance once per year. Food assistance is only available on once every 30 days per individual. Must be a resident of Medina County and meet certain income guidelines. Clients who are accepted for assistance may be placed on a list to receive certain items when they become available.HOURS OF SERVICE: By appointment M,Th: 6:00pm-8:30pm; Tu, W, F: 10:00am-12:00pm (Noon) and 1:00pm-3:00pmFEES: FreeREQUIRED DOCUMENTS: Driver s license or Photo I.D. Proof of income. Proof of address/residency such as current bill or drivers license- rent receipts accepted.
