About Journey Home

Journey Home is the lead agency charged with implementing the Capitol Region s Ten Year Plan to End Homelessness. Agency works in collaboration with local and regional housing, community, and civic organizations to increase the number of supportive and affordable housing units and to end chronic homelessness in the Capitol Region.Elegibility:HPRP provides funding to assist families and individuals who are at imminent risk of homelessness. Must be "imminently at risk of becoming literally homeless within the next 6-15 days":*Being evicted within 6-15 days from private dwelling (own home, or family/friend s home), OR*Being discharged from an institution in which the person has been a resident for more than 30 consecutive days, OR*Living in housing that has been condemned by housing officials and is no longer considered habitable, OR*Fleeing a domestic violence housing situation. IN ADDITION TO ONE OF THE ABOVE:*No appropriate housing options have been identified, and*The person lacks the financial resources and support networks needed to obtain immediate housing or remain in their existing housing.NOTE: If literally homeless or imminently homelessness (within 5 days), refer to homeless shelter. Hours of operation:M-F: 8am-4pm Apply at CRT offices in Hartford or Middletown; Cannot apply more than once in any 18 month period
